Who We Serve

The Holyoke Transitions Academy program is designed to prepare students ages 18-22 with disabilities for their change in status from a student to an emergent adult role

Our Focus Areas

Employment, post-secondary education (functional academics, college, career training), maintaining a home, community involvement, and enhancing relationships.

Learning Tracks

Transition Acadmy offer three inter-connected learning tracks: • Independent/Life Skills Track • College Track • Vocational Track

Our Transition Specialist works closely with the student, family, and team to understand the student’s vision and an activities plan to help meett hat vision.
